NY Highway Use Tax (HUT)

To apply for HUT and AFC credentials, a carrier must file Form TMT-1, Application for Highway Use and/or Automotive Fuel Carrier Certificates of Registration. Include a fee of $15 for each truck tractor, or self-propelled device, and for each trailer, semitrailer, dolly, or other device that transports automotive fuel.

New Mexico Weight Distance Permits

All registrants, owners and operators of motor vehicles traveling New Mexico Highways with a declared gross weight or gross vehicle weight in excess of 26,000 pounds is required by statute to register for a weight distance tax permit.  LEARN MORE

New Mexico Weight Distance Tax Reports

Registrants, owners and operators of most motor vehicles having a declared gross weight or gross vehicle weight over 26,000 pounds and who are using highways in New Mexico are subject to the weight distance tax (WDT) for use of the highways.  LEARN MORE

Kentucky KYU Number

A KYU number is a tax license number issued for the Kentucky Weight Distance Tax.  Under KRS 138.660 (3), any carrier over 59,999 pounds traveling in Kentucky is subject to this tax.  There is no fee to obtain a permanent KYU tax license number. Just fill out the application and submit it by mail. You may obtain a KYU number for your company by completing the Kentucky Trucking Application (TC 95-1) and the Vehicle Additions or Deletions Subject to KYU tax form (TC 95-38)

Application for Oregon Motor Carrier Account

Form 735-9075 – Complete this form if operating trucks with a combined weight over 26,000 pounds or if operating trucks weighing 26,000 pounds or less and hauling for-hire.
